AbbVie Care Patient Support Program Terms and Conditions – advanced Parkinson’s disease

PROGRAM TERMS AND CONDITIONS, SERVICES & ENROLMENT
TERMS AND CONDITIONS

  1. Only Australian residents who have been prescribed and dispensed an AbbVie medication and associated medication delivery system (including pump, tubing, and other ancillaries “Medication Delivery System”) for an indication and dosing schedule registered with the Therapeutic Goods Administration for the management of advanced Parkinson’s disease (“AbbVie Medication), by their prescribing specialist  (“Prescribing Specialist”) are eligible to enrol in and continue their participation in the AbbVie Care Patient Support Program (the “Program”).

  2. The Program is sponsored by AbbVie Pty Ltd (ABN 48 156 384 262) ("AbbVie") and managed and administered by AbbVie's nominated third party providers (“Service Providers”) and authorised AbbVie personnel required to liaise with a patient for customer service and medication stock management ("Authorised AbbVie Personnel"). For the purposes of the Program, Service Providers might be referred to as AbbVie Care Nurse Consultants and/or AbbVie Care Program Coordinators.

  3. Service Providers facilitate the provision of all communications with a patient and their carer through the communication channels offered under the Program. From time to time, a patient and their carer may be sent additional or updated information regarding the Program or their AbbVie Medication.

  4. The Program offers a range of health, wellness and related support services. Participation in the Program is designed to complement and support the prescribing instructions of the Prescribing Specialist or other healthcare professionals (HCP) who are responsible for and assist with the ongoing care and management of a patient’s advanced Parkinson’s disease. All services offered under the Program are optional, allowing a patient to opt into all or a selection of the available services, or opt out of any of the available services at any time.

  5. The Program does not replace the advice of a Prescribing Specialist or other HCP or provide any general nursing services or advice. A patient should speak to their Prescribing Specialist if they have any questions or concerns about their treatment. A patient should also refer to the Consumer Medicine Information available at medsinfo.com.au or by calling AbbVie Medical Information on 1800 043 460 if they have any further questions or concerns.

  6. Information about the interactions a patient has with the Program will be provided to their Prescribing Specialist and their HCP in order for their Prescribing Specialist to manage their on-going care associated with their advanced Parkinson’s disease.

  7. The eligibility of a patient to remain enrolled in the Program is determined by the criteria outlined in clause 1 above, which will be verified on an ongoing basis by Service Providers. AbbVie reserves the right to discontinue the provision of services to a patient under the Program, if a patient does not meet the eligibility criteria and/or in the event that there is any inappropriate behaviour displayed by a patient to AbbVie and/or the Service Providers.

  8. A patient may opt out of any services offered under the Program or the entire Program at any time by calling 1800 230 608 or emailing parkinsonssupport@abbviecare.com.au

  9. If a patient opts out of the Program, personal information collected from them during their participation in the Program may still be held for purposes permitted by law, including reporting purposes. To the extent personal information is retained, it will be handled in accordance with the Patient Privacy Notice.

  10. AbbVie reserves the right to terminate the Program or change and/or cease providing any of the services within the Program with reasonable notice at any time.

 

SERVICES

The Program offers the following services, including general and comprehensive support services.

The Prescribing Specialist and/or HCP in consultation with a patient, or by a patient alone can select the services that a patient may access. If a patient self-enrols, they can only access general support services. Only a Prescribing Specialist or HCP can enrol a patient into comprehensive support services. Patient services may commence prior to or after treatment initiation.

General Support Services - available to all enrolled patients

  • Provision of hard copy and/or digital materials to support a patient during treatment.
  • Medication Delivery System support via the AbbVie Care Nurse Helpline or website
  • Customer Service Support (pharmacy support, ancillary supplies, carrying accessories, pump replacement, batteries, travel support, other order items) via 1800 230 608 or  parkinsonssupport@abbviecare.com.au
  • Scheduled touch point by Service Providers

Comprehensive Support Services – only available for patients enrolled by their Prescribing Specialist and/or HCP

  • Initial AbbVie Care call to explain the Services provided by the Program
  • Education on the procedures involved in initiating therapy with an AbbVie Medication and associated Medication Delivery System
  • Education to support self-management of the Medication Delivery System. This can be offered to a patient and/or other individuals involved in a patient’s care
  • In-person nurse support to a patient and the Prescribing Specialist or HCP with the initiation of an AbbVie Medication and operation of the Medication Delivery System
  • Scheduled home visits and/or phone or virtual support from AbbVie Care Nurse Consultants to support the proper administration and maintenance of the Medication Delivery System, infusion site health and establishing a good infusion management routine
  • A series of SMS and email reminders of information to support the Medication Delivery System and infusion site health

ENROLMENT

Enrolment into the Program:

A patient may enrol into the Program by one the following methods:

  1. Self-enrolment by phoning 1800 230 608 or emailing  parkinsonssupport@abbviecare.com.au into all or a selection of the General Support Services specified above.
  2. Enrolled by their Prescribing Specialist and/or HCP into all or a selection of the General Support Services and/or the Comprehensive Support Services specified above. If a patient is referred for enrolment by their Prescribing Specialist and/or HCP, a patient’s contact information (email address and/or phone number) will be provided to Service Providers, who will contact a patient to complete their enrolment.

In order to enrol in the Program a patient must acknowledge that they have understood and agreed to the AbbVie Care Patient Support Program Terms and Conditions and the Patient Privacy Notice.

PRIVACY NOTICE

Before submitting any information and agreeing to enrol in the Program, a patient should be aware of and consent to the Patient Privacy Notice https://patients.abbvie.com.au/ac-privacy-policy

INFORMATION FOR PRESCRIBING SPECIALIST AND/OR HCP

Service Providers will process Prescribing Specialist and/or HCP personal data to grant them access to the HCP Portal (Portal). Access to the Portal will be subject to our Privacy Notice https://www.abbvie.com.au/privacy.html, which describes how AbbVie  collects and processes  personal data and  data privacy rights.   Providing your personal data to us is voluntary, but if Prescribing Specialist or HCP decide not to provide personal data, AbbVie may not be able to provide access to the Portal.

Access to the Portal is via a secure log in using AbbVie ID.

By registering for the Portal you are agreeing to:

  1. Being able to enrol patients in the Program.
  2. Being able to order Program related materials for your patients.
  3. Receiving emails confirming your patient’s enrolment in the Program.
  4. Receiving emails regarding Program nursing interactions which allows you to access the Portal to view the nursing interaction outcome.

The Program does not replace the advice of the Prescribing Specialist or HCP. The Program is designed to complement and support the prescribing instructions of the Prescribing Specialist or other HCPs who are responsible for and assist with the ongoing care and management of a patient’s advanced Parkinson’s disease. When the Prescribing Specialist or HCP enrols a patient in the Program, Service Providers can provide a patient with either General Support Services or Comprehensive Support Services (as specified above).

This Portal does not replace the clinical care and management of the Prescribing Specialist or HCP for your patient.
